OBTAIN A REPEAT PRESCRIPTION ?

Dispensary Manager - Ms Carol Banham

Repeat prescriptions may be requested by telephoning our repeat prescription number on 784455 between 9.00 am and 12 noon Monday to Friday. Alternatively you may e-mail your request to prescriptions.f81126@nhs.net or place your repeat prescription request in the box situated in the surgery entrance. Please note we need two working days notice to provide repeat prescriptions. If you wish to collect your medicine direct from the chemist please remember to allow extra time for this service.

If your prescription requires reauthorisation, please arrange to see your doctor in good time before your next repeat is due. Patients are issued with one month’s supply of drugs in accordance with Government guidelines. Our dispenser is able to issue medicines for those of our patients who live over one mile from a pharmacy.

OBTAIN A HOME VISIT ?

For home visit requests, please call as early as possible, preferably before 10.00am.

Your call will be passed to one of our nursing team for assessment. If you can get to the surgery, please do so. We rely on the co-operation and good common sense of our patients in this respect. With the facilities at the surgery we can make a much better assessment of your problem and whenever possible, patients will be advised to attend. Home visits are for those too ill to come to the surgery and not for patients for whom it is inconvenient

TEST RESULTS ?

Patients are requested not to contact the surgery for test results. If your test is abnormal you will be notified by one of our team and an appointment arranged for you to see the doctor.

REGISTER WITH THE PRACTICE ?

To register as a patient ask at reception for details. New patients will be invited to make an appointment to discuss their health needs. An initial appointment will be made with our Health Care Assistant for this purpose.

If you wish to choose to see a particular partner for some or all medical conditions please let the receptionist know and she will note this in your medical records. We will always try to ensure that this is possible but there may be occasions when it is not possible.

MAKE A COMMENT/COMPLAINT ?

We try to provide the best services possible but if you wish to make a complaint, please write to the Practice Manager giving full details. We will endeavour to deal with your concerns promptly and efficiently. Please note that we have to respect our duty of confidentiality to patients and a patient’s consent will be necessary if a complaint is not made by the patient in person A copy of our complaints procedure may be obtained from reception.
